Targeted Invites 📨

The Targeted Invites module simplifies the process of identifying and prioritising eligible local cohorts for health checks. By running specialist queries on clinical systems, we take the administrative load off GPs while ensuring that those most in need are prioritised using predictive algorithms.
With automated SMS and postal invites dispatched at the click of a button, this module is designed for flexibility, whether operated centrally or at the site level by the commissioning authority.

Interactive Consultations 💬
Deliver empowering consultations that inspire lifestyle change.
Interactive Consultations allow health check providers to maximise patient benefit and powerfully communicate cardiovascular risk through an industry-leading health check user interface.
During consultations, individuals are supported to fully grasp what their health status means for them through tailored health advice and dynamic graphics.
Designed to support both in-person and virtual consultations, this module ensures complete and accurate datasets, backed by streamlined, pathway-driven processes.

The module also co-ordinates automated e-referrals to lifestyle services, ensuring that your programme’s referral pathways are joined up and streamlined.
Personalised Health Reporting with My Well Record 🌐 (IC uplift)
Give people the tools to change their lifestyle.
Grant individuals access to their health results via a secure online portal: My Well Record.
The digital reports delivered available through My Well Record contain bespoke information based on individuals’ health profiles. All information is communicated in easy-to-understand terms, using colourful graphics and targeted advice.
Signposting can be configured to include lifestyle services and digital support. Commissioners can tailor these follow-up services in line with the range available locally.
Digital Self-Assessment 🏡 (IC uplift)
Enable health checks to be completed digitally and remotely.
Self-assessments allow people to go through a digital health check from the comfort of their own home.
Based on self-reported and clinical data, predictive algorithms are used to determine whether to triage someone for an in-person follow-up.
The functionality enhances the reach, engagement, and digitality of your programme.
Offline templates 📋 (IC uplift)
Deliver health checks in on or offline settings.
Even if you’re delivering health checks in settings where internet availability is unreliable or non-existent, such as in rural outreach initiatives, the system can still support you and your team.
Our offline templates functionality ensures that datasets can be collected using an offline interface that doesn’t compromise patient engagement.
